Open Enrollment & Intra District Applications available now photo

for the 2021-2022 school year
are now available!


Parents/Guardians must submit an application for each school aged child, each year.  Applications are accessible by clicking on the link below. Please complete the appropriate form by April 30th.  You will be notified of your child's status, via regular mail,  by July 15, 2021.

Reopening Plan-Fall 2020
Medication Pick-Up nurse

If your child has medication or medical supplies in the school clinic, please arrange for someone (parent, grandparent etc.) to pick up the medication/supplies at your child’s school building by 3:00 p.m., Wednesday, May 26, 2021. 

Please note: Students are not permitted to take medication/supplies home with them. 

If items are not picked up by this date, the medication and medical supplies will be disposed of on Friday, May 28th.

Mandated School Immunizations 2021-22 School Year

Updates on immunization requirements for the 2019-2020 school year -  Sacramento City Unified School District

Immunizations Required for Incoming 
Kindergarten Students

5 doses DTaP vaccine
4 doses Polio vaccine
3 doses Hepatitis B vaccine
2 doses MMR vaccine
2 doses Varicella/Chicken Pox vaccine

 

Immunizations Required for Incoming 
7th Grade Students

If your child is currently up to date on all immunizations he/she will need 2 vaccinations prior to beginning 7th Grade.    
1 dose Tdap vaccine
1 dose Meningococcal vaccine 

 

Immunizations Required for Incoming 
12th Grade Students

If your child is currently up to date on all immunizations, he/she will need a second dose of the Meningococcal Vaccine prior to beginning 12th Grade. 

(Note:  If the first dose of the Meningococcal Vaccine was administered after the child’s 16th birthday, only one dose is required).
