2017-2018 Physical Forms

I love sports graphic    2017-2018 Preparticipation physicals can be accessed by clicking on "School Forms" on the high school or middle school site.  Students must have a current physical turned in before the first day of try-outs for their chosen sport.  These are the forms that you take to your doctor's office to have them complete and sign before turning in to the athletic department.  

Mandatory Fall Sports Parent Meeting fall sports

Tri-Valley will hold a meeting for ALL student athletes and their parents, Monday, August 7th, 6pm in the TVHS Cafetorium.  If you have a student in grades 7-12 participating in football, volleyball, soccer, cross country, cheerleading, or golf, we need you at this meeting.  All paperwork for the season will be completed and turned in.  This will eliminate numerous individual sport meetings by covering rules, athlete code of conduct, etc. at one meeting.  Bring a pen and contact phone numbers to complete your emergency medical authorization paperwork.  Thank you, in advance, for helping us to streamline this process!
